This autograph is on a Nike elite ball and is signed on the panel just above the Nike logo. This is pretty cool because Jordan has his shoe company through Nike so makes it that much more unique. This has to be by far one of the nicest, cleanest MJ signatures around you can see he took his time to sign this ball right!!!

This autographed item has been certified authentic by upper deck authenticated (UDA) In which MJ signs exclusively with them and includes their tamperproof hologram located on the basketball. Unfortunately, the paper COA card has been lost and does not come with this item. We have reached out to Upper Deck and had it confirmed of its authenticity under its serial number (#uas15415) And comes with an email from an upper deck representative in their customer service department verifying it is authentic (See photos). This paper does come with the ball.
